Insurance, billing and new patients

Everything practical in one place. If something here is unclear, call the office — we would rather answer a question than have you guess.

Your first visit

Plan for a longer appointment than usual. The point of a first visit is to build an accurate picture of your history rather than to address a single complaint.

Please bring

  • A photo ID
  • Your insurance card
  • A list of every medication and supplement you take, with doses
  • The name and address of your previous primary care practice
  • Names of any specialists currently involved in your care

New patient paperwork: TODO — link your intake forms here, or say that forms are completed in the office. Do not host fillable forms that email unencrypted health information.

Insurance we accept

We are in-network with most major PPO plans and a number of HMO and Medicare plans.

TODO — replace with your actual contracted plans. List them by payer and plan type, and add the date the list was last reviewed. An out-of-date network list is the single most common complaint patients have about clinic websites, and it creates real billing disputes.

If we are out of network

We will tell you before your visit, not after. TODO — state your self-pay rate for a standard visit and a new-patient visit. Practices that publish this get more calls, not fewer.

Everyday requests

Prescription refills

Ask your pharmacy to send the request electronically — it is the fastest route. Allow two business days. Some medications require a visit before we can refill them.

Test results

Results are released to the patient portal as they arrive. We call you directly about anything that needs discussion or changes your plan.

Medical records

To request a copy of your records, or to have them sent elsewhere, call the office. We will need a signed authorisation. TODO — state your turnaround and any copying fee.

Frequently asked

Are you accepting new patients?

Yes — both offices are accepting new patients. You can request an appointment online or call us, and we will usually get you in within TODO weeks for a first visit.

Which insurance plans do you accept?

We accept most major PPO plans and a number of HMO and Medicare plans. Because networks change, the accepted-plan list on our Patient Information page is the authoritative one — and it is always worth a quick call to confirm before your first visit.

Can I be seen the same day if I'm sick?

We hold same-day slots at both offices each morning and afternoon for established patients. Call as early in the day as you can. If we're full, we'll tell you plainly and help you find care rather than leaving you waiting.

Do I have to pick one office, or can I use any of them?

You are welcome at either one. Your chart is the same in both, so if Bolingbrook is closer to work and Aurora is closer to home, use whichever fits the day.

What should I bring to my first appointment?

Photo ID, your insurance card, a list of the medications and doses you currently take, and the name of any specialist or prior practice we should request records from.

How do I get a prescription refilled?

The fastest route is to ask your pharmacy to send us a refill request electronically. You can also message us through the patient portal. Please allow two business days.

Is this an urgent care or an emergency room?

Neither. We are a primary care practice. For a possible heart attack, stroke, serious injury, trouble breathing or any life-threatening emergency, call 911 or go to the nearest emergency department.

After hours and urgent needs

During office hours, call us first — we can often handle an urgent problem faster than an urgent care can. Outside office hours, calling the main number will reach the on-call service.

Call 911 for a medical emergency, including chest pain, symptoms of a stroke, difficulty breathing, severe bleeding, or any sudden severe symptom.

For mental health crisis support, call or text 988 at any hour.
